Institutional Understanding for Healthcare

It is the one clinical process with no artefact - and the one you are accredited on.

A watch item raised at handover, and never mentioned again.

The shift changed. The clinical nuance did not transfer.

dhi Healthcare Clinical Governance Dossier Artifact

Why nobody catches this today

A patient is transferred to another unit at 03:00. The handover happens verbally, vital signs are entered in the EHR, and the discharge plan is updated.

What does not transfer is the unspoken clinical concern - why a particular test was deferred, the borderline response to treatment that didn't meet escalation threshold, or the family's nuanced instruction. That remained in the room.

Twelve hours later, an adverse event occurs, and the care team must reconstruct the clinical reasoning from disconnected telemetry and incomplete notes.

What else dhi looks for

Watch Item Unrecorded Across Shifts

A watch item raised at nursing handover, never recorded in the subsequent shift care plan.

Escalated Vital Trend Without Review

An escalated vital sign trend discussed during morning rounds with no dated clinical review entered.

Verbal Discharge Instructions Discrepancy

Discharge instructions given to family on a call that contradict the documented discharge summary.

Verbal Care Pathway Alteration

A care pathway altered verbally across multidisciplinary team discussions with no formal note on file.

What dhi does not have a view on

dhi has no view on whether a clinical decision was right. It has no view on whether an observation warranted escalation, or whether a plan is appropriate.

Every finding reports what was said, and what was not said next.

A handover declared SBAR and no recommendation was recorded is a fact about the record. This patient should have been escalated is a clinical judgement, and dhi cannot produce one - not because it is disabled, but because no such output type exists.

That is not a limitation to be relaxed as the product matures. It is the reason a hospital can deploy this at all.

What dhi will not do here

No clinical judgement, ever. No diagnosis, no treatment recommendation, no risk score about a patient.

No finding about a clinician. Findings describe encounters, pathways and processes. If clinicians believed dhi reported on them, they would stop speaking candidly at handover - and handover is the input the whole thing depends on.

And no clinical values are stored. References to your records, never their contents.
